Subcutaneous injections of carbon dioxide (CO2) can safely and successfully treat cosmetic concerns such as skin laxity and fatty deposits that may remain following liposuction, as well as psoriasis and hair loss, says an expert based here. Additional uses for this treatment . called carboxytherapy . include stretch marks, scars and cellulite, he says.

The Carboxytherapy consists in the therapeutic use of carbon dioxide (CO2) on its gaseous state, either transcutaneous or by subcutaneous injection.

This therapy was born in France, in the thermal waters station of Royat, near Clermont Ferrand. There a group of cardiologist ascribed to the hospital of Clermont Ferrand, began to treat patients with peripheric organic and functional arteriopaties (Atherosclerotic, Buerger's disease, Raynaud's disease, etc). In 1953 the cardiologist M.D. Jean Baptiste Romuef, published a paper about his 20 years of experience in the subcutaneous injection of CO2. Afterwards, the Parisian cardiologist M.D. Jerome Berthier, along with M.D. Luigi Parassoni from Gallarate, started its application on patients with cellulite.

Hemoglobin, the main oxygen-carrying molecule in red blood cells, carries both oxygen and carbon dioxide. However, the CO2 bound to hemoglobin does not bind to the same site as oxygen. Instead, it combines with the N-terminal groups on the four globin chains. However, because of allosteric effects on the hemoglobin molecule, the binding of CO2 decreases the amount of oxygen that is bound for a given partial pressure of oxygen. The decreased binding to carbon dioxide in the blood due to increased oxygen levels is known as the Haldane Effect, and is important in the transport of carbon dioxide from the tissues to the lungs. Conversely, a rise in the partial pressure of CO2 or a lower pH will cause offloading of oxygen from hemoglobin, which is known as the Bohr Effect.

La carboxytherapie (CDT, Carbon dioxide therapy, Carboytherapy, Carbotherapy, Carboxy-mesotherapy) connait, depuis le début des années 2000, une popularité croissante parmi les médecins esthéticiens. La technique d.injection de CO2 médical par voie sous cutanée a été inventée eu France dans les années 30 aux cures thermales de Royat dans des applications initialement vasculaires. Les bains riches en CO2 thermal avaient en effet pour propriété de soulager les artériopathies des membres inférieurs par effet vasoactif et effet Bohr. Les médecins thermalistes ont expérimenté des injections sous cutanées directes du même gaz avec des effets médicaux potentialisés. On retrouve dans la littérature des applications thérapeutiques dans la prise en charge des cicatrisations des ulcères à composante vasculaire. Les applications esthétiques des injections de Gaz CO2 ont été publiées par les Italiens (D.Aniello et Brandi, Université de Sienne) dans le cadre de la lutte contre l.effet peau d.orange de la cellulite. La technique est actuellement très répandue dans les pays d.Amérique du Sud, notamment au Brésil, avec une gamme d.indications élargie : cellulite, graisses localisées, rides, vergetures, cernes péri-oculaires, rel.ement cutané.
